The Treatment of Premature Ejaculation (PE) There are three good reasons to treat PE: 1. We have found that it is often a life long problem until a plan for treatment is undertaken. It does not usually go away without help. 2. Once you have treated it and achieved control, we have found that this control lasts and is not forgotten. I often compare this to riding a bike. Once you have the confidence to do so you do not forget. 3. I personally have found that long term sufferers of PE are more likely to develop ED later in life. Medical Sublingual SprayDue to its simplicity, safety and effectiveness this method has become extremely popular in recent years for the treatment of PE. One or two sprays of the medication applied under the tongue half an hour before intercourse is all that is needed for up to 12 hours of cover. At first this method may seem to be nothing short of a miracle, but in fact it has a very solid scientific foundation. For years we have known that certain types of antidepressants inhibit ejaculation but have been reluctant to use them due to their side effect profile and the need for daily use. By placing certain antidepressants in a specific medium, we found that we were able to lower the dose to sub therapeutic levels where by the side effects experienced by normal antidepressant were mostly eliminated. To put this in simple terms, usually a very low dose of an antidepressant applied under the tongue will inhibit ejaculation without producing the side effects seen with normal antidepressants. To determine the exact dose needed, the patient is weighed first. It is recommended that this spray be used 2-3 times per week for a period of 6 months. After this time most men will have learnt to control their PE without the need for further medication. This medication is available by prescription and subject only to one of our doctors approvals. 
Intracavernous Pharmacotherapy (ICP)This treatment uses a combination of four vasodilators which when used, provoke an erection that usually lasts for 40-60 minutes. This erection will not go down sooner even if you ejaculate. We encourage you to continue to make love while using this medication for 30-40 minutes. In time, the hypersensitivity of the head of the penis will diminish and you will get used to making love for longer. Non medical treatments for PEThese include psychological counseling or education, the use of anaesthetic gels or sprays, herbs and supplements and various exercises. I have found that these methods produce poor results and as such we usually do not promote their usage in our clinics.
